This form is completed by an applicant to give the VA the right to disclose a person`s personal beneficiary or to request information from third parties. As mentioned above, none of these forms directly affect the decision on a pension entitlement, but if they are in place, you can manage the right more effectively. The person named on this form must be copied in all correspondence issued by the VA to the applicant. Form VA 21-4142 – Authorization and Consent to Disclose Information to the Department of Veterans Affairs (VA) (243K) The Veterans` application and authorization to disclose medical records or health information, or ”AV Form 10-5345”, is a document that allows the collection of treatment records for physicians or health care providers after their active service has ended if they have already been processed in an institution for Veterans.
